This cancellation form is for all other types of services such as Hosting, Virtual Email, or partial service termination requests (i.e. you would like to cancel your Domain Hosting but not your Internet Access). For DSL, Dialup, or Wireless accounts, please use the appropriate form here .

For your security, the information below is required. You may print and mail the form if you prefer. If the information below is not accurate, or does not match our records, your account will not be closed and we will assume the authorized account holder did not submit this request.
